How to start a vote in CS:S

Type callvote changelevel de_dust2 in your game console to start a map vote; callvote alone lists issues. Needs sv_allow_votes 1.

callvote

Starts a vote on an issue such as kicking a player or changing the map.

callvote <issue> [option]

Vote to switch to Dust2.

What it does

Run it without arguments to list the available vote issues. Needs sv_allow_votes 1 on the server.

Run it on your server

On a SLASHPLAY server, type it in the Console tab of the server page; the reply shows right under it. A value set there lasts until the server restarts. To keep it, add the line to cstrike/cfg/server.cfg. New to this? Start a CS:S server first.
